Team Member Job Descriptions - TTransportation Coordinator

Position Summary:

The primary purpose of this position is to plan, organize, develop and direct the overall operation of the Transportation Department in accordance with current federal, state and local standards, guidelines, and regulations that govern our facility, and as may be directed by the Administrator.

Risk Exposure Categories:

  • 1 - Tasks may involve exposure to blood/body fluids.
  • 2 - Tasks do not involve contact with blood/body fluids but could result in performing a Category 1 task.
  • 3 - Tasks do not involve any risk of exposure to blood/body fluids.

Position Functions:

Plan, organize, develop, implement, and direct the Transportation Department, as well as its programs and activities. 3
Develop, methods for coordination of transportation services with other resident services to ensure the continuity of the resident's total regimen of care. 3
Develop, maintain, and periodically update written job descriptions for transportation team members. 3
Develop and maintain written transportation policies and procedures. 3 Interpret the department's policies and procedures to team members, residents, visitors, government agencies, etc. 3
Assist the transportation team members in the development and use of departmental policies, procedures, equipment, supplies, etc. 3
Perform administrative duties (completing paperwork, evaluations, reports, etc.). 3
Recommend program adjustments to Administration as necessary. 3
Submit accident/incident reports within twenty-four hours of the occurrence. 3
Recruit, select and orient new transportation team members. 3
Determine departmental staffing needs while remaining in budget. 3
Schedule team members to meet the needs of the facilities and residents. 3
Coach team members on compliance and performance and issue disciplinary action as needed. 3
Conduct regular competency testing of team members. 3
Maintain a productive working relationship with other department supervisors and coordinate transportation services to assure that care and services can be performed without interruption. 3
Assure transportation team members complete ongoing education and training. 3
Develop and maintain maintenance prevention sheets. 3
Ensure residents safely arrive at their appointments as scheduled. 2
Identify safety hazards and emergency situations, initiating corrective action. 2
Attend all in-services and complete assigned Relias courses by set deadlines. 3
Maintain confidentiality of resident and facility information. 3
Observe all facility safety policies and procedures. 3
Present to work with a clean, neat appearance to represent the organization in a professional manner. 3
Report to work as scheduled, consistently demonstrating dependability and punctuality. 3
Accept assigned duties in a cooperative manner. 3
Follow Resident Rights policies at all times. 3
Assume responsibility for the resident's safety under their supervision. 2
Ensure wheelchairs are safely tied down. 2
Use appropriate lifting procedures when transferring residents. 2
Maintain the vehicle's gasoline level at a half a tank or above. 3
Accurately maintain the vehicle's mileage log. 3
Report repairs to maintenance or the Director of Transportation in a timely manner. 3
Communicate effectively between Winning Wheels and providers. 3
Ensure new appointments are recorded. 3
Display effective time management skills (arrive on time, schedule accordingly). 3
Provide assistance to the resident during their appointment and transportation. 2
Report necessary information to the nurse on duty. 3
Process medical transport requests (Trip Slips). 3
Process weekly reports. 3
Assure that resident's personal property is handled appropriately. 3

 

Working Conditions:

Works in nursing environments, offices, and company vehicles.
Sits, stands, bends, lifts, and moves intermittently during working hours.
Is subject to frequent interruptions.
Is involved with residents, team members, visitors, government agencies, etc. under all conditions and circumstances.
Is subject to hostile and emotionally upset residents, family members, etc.
Attends and participates in continuing education programs.
Is subject to falls, burns from equipment, odors, etc. throughout the workday.
May be exposed to infectious waste, diseases, conditions, etc. including the AIDS and Hepatitis B viruses.

 

Experience and Education Requirements:

Minimum, High School Diploma or Equivalent.
Valid, current driver's license in the State of Illinois.
On the job training provided.

 

Position Requirements:

Must be able to read, write, speak and understand the English language.
Must possess the leadership ability and willingness to work harmoniously with and supervise professional and non-professional team members.
Possess the ability to make independent decisions when circumstances warrant such actions.
Must be able to plan, organize, develop, implement, and interpret the programs, goals, objectives, policies, procedures, etc. of the Transportation department.
Possess the ability to deal tactfully with others.
Be knowledgeable of transportation practices and procedures.
Must have patience, tact, a cheerful disposition, and enthusiasm.
Must have the ability to relate information concerning a resident's condition.
Must be able to move intermittently throughout the workday.
Possess sight/hearing sense or use prosthetics that will enable these senses to function adequately.
